The Assisted Living Waiver Program (ALWP) is a Medi-Cal program designed to help elderly and disabled individuals transition from nursing homes or other institutional settings into assisted living facilities with supportive care services. Through ALWP, eligible participants receive assistance with personal care and activities of daily living (ADLs), medication management, nursing support for chronic medical and mental health conditions, and access to social and recreational activities within an assisted living setting. As a less restrictive alternative to nursing homes, ALWP allows individuals to live in a more home-like environment while still receiving the necessary care and support to maintain their well-being and independence in the community.
The following is a list of some of the services that can be provided to ALW participants. These services may be provided in a Residential Care Facilities for the Elderly (RCFE) or by a licensed Home Health Agency to residents in public housing.
Care Coordination: These services include identifying, organizing, coordinating and monitoring services needed by participants.
Nursing Facility Transition Care Coordination: These services help transition participants from a nursing home to the community.
To be eligible to receive services as an ALW Participant, an individual must meet the following ALW eligibility criteria:
Income
While the Assisted Living Waiver Program has no income limits, participants must be eligible for full-scope Medi-Cal coverage with no Share of Cost. In rare circumstances, people who exceed the Medi-Cal income limits can still be eligible for the Assisted Living Waiver Program.
