Enhanced Care Management (ECM)

How does Enhanced Care Management (ECM) work?

• A care team works with the member to create a care plan. 
• The care team coordinates care and services from different systems, such as physical, behavioral, dental, developmental, and social services. 
• The care team helps members find doctors and get appointments. 
• The care team helps members manage their medicines. 
• The care team helps members get follow-up services after leaving the hospital. 

To be eligible to receive services as an participant, an individual must meet the following  eligibility criteria:

• Currently experiencing homelessness
• At risk for avoidable hospital or eating disorder (ED) use
• Have serious mental illness or substance use disorder
• Transitioned from incarceration within the last 12 months
• Have chronic disease, intellectual or developmental disability, traumatic brain injury, HIV, pregnancy, or postpartum
